He changed his name to Sam Aurelius Milam.  No one that I've asked about it knows when.  I remember a conversation that I had with him about it when I was quite young.  I was riding somewhere with him in his car and noticed that his cuff links has his first name on them.  I commented about it and that's when he explained it to me.  He pointed out that it was also his initials.  He said that he'd changed his name because he wanted his initials to spell his first name.
